By digitally signing this document, I agree that:

The City of Woodburn has the right and permission to forever use and reuse and publish, in any manner at all, videos and/or photos taken of me, in whole or in part, modified or altered, either by themselves or in conjunction with other videos, in any medium or form of world-wide distribution, and for any purposes whatsoever, including without limitation all promotional, print, online and advertising uses, non-commercial or commercial display, broadcast, exhibition of the final production and other trade purposes.

The City of Woodburn is permitted to extend these rights and permissions to any person, business, organization, community or media.

I waive the right to inspect or approve the finished product or copy of any use thereof.

The City of Woodburn retains all copyrights to photos and/or videos.

I hereby forever release and discharge The City of Woodburn and the City’s respective representatives, successors and assigns, specifically The City of Woodburn and its clients, from any and all claims, actions and demands arising out of or in connection with the use of these videos and/or photos.

I acknowledge that this release document was signed willingly, and I certify that I am not a minor and I am free and able of giving such consent. If an undersigned is signing this form on behalf of, or as an agent or employee of the person photographed or recorded, the undersigned warrants that he/she has full authorization to do so.
